 Before moving loads with ALS, Inc., we will need you to fill out and fax back to us the following forms and items:
 Our fax number is 479-587-8040.

  1. ALS Brokerage Agreement. 
  2. A copy of your Operating Authority.
  3. Proof of insurance (cargo and liability).  (Our insurance minimums are as follows: Cargo $100,000 and liability $1,000,000).  Before settlement of payment is made we will need your insurance company to send us a certificate of insurance with Accord Logistics Solutions, Inc. as the certificate holder and we must have 30 days notice of changes or cancellation.
  4. Completed W-9.
  5. ALS Carrier Profile Sheet.
  6. The Rate Confirmation will be sent to you after the above items are received and reviewed.  After you receive and review the Rate Confirmation, please sign and fax back to us.
  7. ALL DRIVERS MUST CALL US FOR DISPATCH.

 

For prompt payment, here are some tips.

  1. Be sure your insurance company has sent us an original insurance certificate with our name as certificate holder.
  2. Be sure your freight bill and all paperwork references the ALS reference number.
  3. Be sure that the bill of lading has the receiver’s signature.  Settlement of payment will not be made without a receiver’s signature.  If we have to obtain a signature, we will charge an administrative fee. 
  4. Send your invoice with the required documents to:
